Adoption Home Assessment Application

What is the purpose of this form?

People interested in adopting children through the Missouri Children's Division (CD) may use this form to begin the adoption assessment process. The CD social worker will use this information to begin exploring the family's individual characteristics. At this time the Children's Division uses this form only for gathering information about persons interested in adopting Children's Division children. Although the form title includes an assessment of Foster Parents, it is not used for Foster Parents. Please contact the local CD office if you are interested in Foster Parenting and not adoption.

How can you use this form?

Individual items on this form are self-explanatory. If you have questions concerning this form or its use, or wish to request a paper copy of this form, please call the Foster Adopt Hotline at (800) 554-2222 or call the CD office in the county where you live and ask to speak with a children's service worker or supervisor.

Where to Send Forms:

Assessment forms should be sent in an envelope marked "Confidential" your local county Children's Division office.

What happens once you mail the application form?

Once received in the Children's Division Central Office, this confidential information will be sent to the CD office in the county where you live. A social worker in that office will initiate action on the application within 30 days of its receipt by CD.
